URGENT CARE

  • Please do not walk-in for an emergency appointment today. Please visit minor injuries, call 111 or 999 depending on the severity of your needs
  • If you suspect a stroke, heart attack or come across someone who has fallen, please dial 999
  • We are not an emergency service 

WALK-IN/SAME DAY APPOINTMENTS

  • We are not a walk-in centre. Please do not walk-in and wait for an appointment.
  • Appointments must be booked in advance
  • We have a limited number of same day appointments which are used by the duty doctor for urgent matters.
  • If you feel you need an urgent same day appointment, please submit a medical request on Rapid Health. The duty doctor monitors this inbox. For more information on how to submit this, please see the section above

TO BOOK AN APPOINTMENT

  • Use Rapid Health to request an appointment. The responses to your request will vary, depending on your needs. You may be offered a link to book an appointment or admin may book for you and will reply with booking details. 
  • Call the surgery 01273 492255 
    • Please note that all calls (inbound and outbound) are recorded for safety and training purposes. For more information on this, please see our Master Privacy Notice on our GDPR and Privacy page.  

Please check your booking confirmation to see which type of appointment you have, and if you need to come in to the surgery.

Please read the following:

  • Rapid Health is our booking triage system that puts YOU in control of your healthcare journey.    
  • If you are filling this out on someone else's behalf, the form must be in the name of the person who it concerns, not your own as it needs saving to their record

Read the examples to decide which is most appropriate pathway for your needs. It will take you to a new page where you will submit your request. You may be asked to resubmit your request if you choose the wrong option

Submit a Medical Request

MY REQUEST IS URGENT AND CLINICAL

MY REQUEST IS NOT FOR A NURSE 

Your request will be reviewed by the Duty Doctor who monitors requests daily via the Rapid Health inbox

 

See some Medical request examples:

  • I am in pain and need an appointment/ i'd like to inform the Duty Doctor
  • I need pain relief from the Duty Doctor
  • I have a new health concern or new symptom and need an appointment/ i'd like to inform the Duty Doctor
  • I need to book an Orthopedic/ Physio appointment with Ryan

 

Enter your main symptom and Search.

Read the options carefully and choose the best option for your needs.

Answer the questions the best you can.

Depending on your needs, you may be asked to go to A&E instead.

If suitable, you will either be offered an appointment or your request will go to the duty doctor to review.

Submit an Admin Request

MY REQUEST IS NOT URGENT 

Your request will be reviewed by Admin Staff who monitors requests daily via the Rapid Health inbox

 

Some Admin request examples:

  • I need to book a Nurse appointment (use the 'Ask a question' option)
  • I need a NON URGENT, ROUTINE GP appointment
  • I need to request / book a blood test
  • I need to book Vaccinations
  • I need Doctor's letter
  • I need a referral (requires an appointment)
  • I need to request a Fit Note
  • I have a NON CLINICAL question to ask
  • I need contraception advice / to discuss my current treatment
  • I need to update my details (contact detail or informing a change of address)

 

Have you tried looking on the NHS App to:

  • Check blood test results 

  • Find reports/letters

  • Check/Order repeat medication 

 

Have you used the Health Kiosk in reception?

  • Pill check 

  • Prior to your HRT review

  • Blood Pressure readings

  • Height and Weight

Submit a request for an Under 16

Please use the Child Request (Under 16) option for Medical concerns and Admin queries

 

The request must be made using the name of the child it concerns, not your own as it needs saving to their record. You will be asked to resubmit this if incorrectly submitted

NHS 111 Online

111 online is a fast and convenient alternative to the 111 phone service and provides an option for people who want to access 111 digitally. 

Your needs will be assessed and you will be given advice about whether you need:

  • Treat yourself at home
  • Go to a Primary Care Centre

If you need face to face medical attention you may be asked to attend a Primary Care Centre.

All calls are free from landlines and mobile phones.

Please do not ask to see a doctor 'Out of Hours' unless you genuinely cannot wait until the surgery re-opens.
